LATE PICKUP FEES: I understand that the following fees are assessed for late pickup after 6:00 p.m. Monday-Thursday and after 5:30 p.m. on Friday for the Extended Day Supervision: 5-15 minutes - $25, 16-30 minutes - $50. These fees will be collected by credit card on the day of service.
I understand that it is required by the Georgia Department of Early Care and Learning that my child not be released from EDP without a signature from the person authorized to pick my child up. I acknowledge it is my responsibility to keep my child’s records current to reflect any significant changes as they occur, e.g., telephone numbers, work location, emergency contacts, child’s physician, child’s health status, immunization records, etc. The facility agrees to keep me informed of any incidents, including illness, injuries, reactions to medications, etc., which pertain to my child. I authorize the Extended Day Program Supervisors to obtain emergency medical care for my child when I am unavailable. I agree to abide by the policies.
We are advising you that Bright from the Start, Georgia Department of Early Care and Learning, is exempt from licensing for all the schools programs. By obtaining a religious faith-based exemption in 2007, The Davis Academy is not required to be licensed as long as the school is accredited by accreditation agencies. The school is accredited by SAIS and the AdvancEd Accreditation Commission. The Davis Academy carries liability insurance for the Extended Day Program (EDP) as necessary.
